MR Perfusion Imaging

Case 1: Recurrent Glioma

Case History:

39 year female with recurrent oligodendroglioma. Post-surgery / RT / TMZ in 2009. Recent MRI shows an enhancing lesion. FDG PET/MRI is done for evaluation.


A) Axial FLAIR, B) FDG PET, C), & D) Axial and Sagittal Post-contrast T1 reveal the post-op cavity with altered intensity in the adjoining area showing ill-defined punctate enhancement. It shows mild FDG uptake not anyway different from the normal brain parenchyma, E) Relative CBV map shows increased perfusion suggesting the possibility of recurrence more likely.

Case 2 : Radiation Necrosis

Case History:

In another patient treated for recurrent right frontotemporal oligoastrocytoma developed an enhancing lesion in follow up MRI. Simultaneous PET/MRI was done for evaluation.


A) Axial FLAIR, B) Axial Post-contrast T1 reveal post-op cavity with enhancing nodularity along the posterior margin of the cavity, C) FDG PET/MR fused image does not show any uptake, D) Relative CBV map shows no increased perfusion suggesting possibility of radiation necrosis more likely.
