Unsuspected Osseous Metastasis in Stage II Cancer


Case History:

48 year old female with HPE proven invasive Lobular carcinoma of the left breast. WB PET /MRI was done for staging workup. XRM MLO (A) CC (B) revealed asymmetric spiculated density in the upper inner quadrant of the left breast.


Findings:

MR mammography as a part of WB PET MRI revealed an enhancing lesion corresponding to the site of mammographic density measuring 2.6 cm in its largest dimension(T2) on post-contrast FS GRE axial (C) Sagittal(D) and Increased FDG uptake on their corresponding PET MR fused images (F, G).

Enlarged avid level I axillary nodes are seen ( E, H ) PET MR stage T2 N1a


Unsuspected osseous metastasis was seen in the left iliac bone (arrows in Axial STIR, A and B), S1 (arrowhead in Sagittal STIR, C), left femur and left ischial tuberosity with no/minimal FDG uptake (PET MR fused images D, E, F) upstaging stage 2 disease to stage 4.
