Progressive Supra Nuclear Palsy

 

Case History:

68 year old female with a history of recurrent falls and abnormal eye movements.

Findings:

 

Axial T2W (A), FLAIR (B) and sagittal T2W (C) images at the level of midbrain showing reduced anteroposterior midbrain diameter at the level of the superior colliculi.Sagittal T2W image (D) of the brain in a magnified view showing flattening of the superior aspect of the midbrain (arrowhead) which is normally upwardly convex: Humming Bird Sign.

 

Qualitative and quantitative comparative analysis to a standard age-matched database using the SCENIUM software shows marked FDG hypometabolism involving bilateral frontal cortices (R>>L) including the pericingulate regions, relative hypometabolism in bilateral lateral temporal regions, bilateral corpus striatum (R>>L) and brainstem including midbrain and pons with relative FDG hypometabolism in bilateral cerebellum.
