Radiation Necrosis on FDG-PET/MRI


Case History:

73 year old male with right tempo-parietal GBM with post-op Radiation Therapy. Now recurrent enhancing lesion on follows up MRI. For evaluation with FDG PET/ MRI.

Findings:

 

Axial sections of the brain showing a heterogeneously enhancing lesion (A) in the right temporoparietal region in the operated bed with no significant perfusion (D), no diffusion restriction(E), no FDG uptake(C) and no significant Choline on MR spectroscopy (F) favoring Radiation necrosis more than Recurrence. Note: Simultaneous acquisition of multiple parameters in a single study facilitates the placement of ROI on the CEMR images to be overlaid on the PET and other MR images which ensures accurate spatial localization for quantitative evaluation of all parameters.
