Tumor Recurrence on FDG PET/MRI

Case History:

49-year-old female, a case of frontoparietal Gr III oligo astrocytoma post-op/RT with a new enhancing lesion on follow up MRI.

Findings:

 

(A) Axial T1W post-contrast image shows enhancing lesion in the right periventricular lesion. Freehand ROI was placed on the enhancing lesion, copied and pasted on the PET(B), PET/MR fused (D), ADC map(E) & shows corresponding increased FDG uptake (arrows in B, C), increased perfusion (arrow in D), restricted diffusion (arrow, E), multivoxel 1H-MRS(F) reveals increased choline: creatine ratio suggesting tumor recurrence.

Note : Precise localization of a small ROI on all images identifies the lesion.
